Complaint Requirements

Anyone can submit a Medicaid complaint to the Agency. If your issue involves a Medicaid Managed Care Plan, we encourage you to reach out to the Plan directly for assistance. For enrollee issues, you can find the phone number for each plan on the General Plan Contact Information list. For provider issues, you can find the phone number for each plan on the Plan Complaint Contacts for Providers list.

If you are unable to resolve your issue directly with the Plan, you may submit a complaint to the Agency by using the Florida Medicaid Complaint Form, or by calling the Medicaid Helpline at 1-877-254-1055 (TDD 1-866-467-4970). Staff are available to assist you Monday through Friday, 8am-5pm EST.
Any problem that a recipient or provider is having with Medicaid can be reported as a complaint and all submissions are reviewed by Agency staff. Some examples are:
  • Missed or late services
  • Getting a service authorization
  • Problems getting care
  • Claims or payment Issues
  • Unable to find a participating provider
  • Unhappy with services

If your complaint is regarding Medicaid eligibility or the application process, please contact the Department of Children and Families ACCESS Program.

If you need to request copies of your medical records, please contact your health care provider directly.  If you believe that the provider is not complying with your request, you may file a complaint with the Department of Health.  For all other requests, contact the Agency's Public Records Office.
