Nursing Home Federal Reporting

Nursing Homes are required by the federal government specifically section 483.12, CFR to report and investigate all allegations of abuse, neglect, exploitation, or mistreatment, including injuries of unknown source, and misappropriation of resident property to the State Agency.

Nursing Homes must submit a 2-Hour/Immediate Report and a follow up Five-Day Report to the Agency for Health Care Administration.

  • The Immediate Report should be submitted as soon as possible, but no later than 24 hours of discovery of the incident. If the events that cause the allegation involve abuse or result in serious bodily injury, they must be reported within two hours after the allegation is made.

  • The Five-Day Report must be submitted within 5 working days from the date of the incident. Any reports received after that time will be considered out of compliance with the Federal Regulations.

Benefits of using this system include:

  • Real-time electronic submission, tracking, and confirmation of all reports submitted on behalf of the facility by authorized users. This is helpful information to have before and during survey.

  • Multiple facility users have access to the view and submit reports.

  • Suspicion of crime against the elderly (Elder Justice Act) reports may also be submitted using this system. The facility should notate that within the report.

Once at this site, you may submit a report if you are already a registered user.

To register as a new user click on NEW USER?, to create your Online User Agreement, ID, and Password.

Once a user submits their Online User Agreement, the user and the Nursing Home Administrator will need to sign the printed out User Agreement. Be sure to send in the User Agreement, along with a current copy of the Facility License, to the fax number(s) provided on the User Agreement.

The online system will allow up to FIVE active users. Facilities must keep their list of users updated to allow for efficient submission of reports.
