Waiver Renewal - Familial Dysautonomia Waiver

The Agency is requesting a renewal of the Familial Dysautonomia Waiver to: (1) extend the waiver period for an additional five years, (2) update the quality performance measures, (3) revise provider qualifications for support coordination, (4) change the name of the contracted vendor, (5) revise the consumer price index, (6) update the public notice process and (7) include the FD Transition Plan in compliance with the HCB Settings Rule.
