About EHR

An Electronic Health Record (EHR) is a longitudinal electronic record of patient health information including patient demographics, progress notes, problems and medications, vital signs, past medical history, immunizations, laboratory data and radiology reports.

EHRs can bring a patient’s total health information together in one place, so care is better coordinated. With an EHR, the incidence of medical error can be reduced by improving accuracy and clarity of records, reducing duplication of tests and delays in treatment, and helping patients stay informed to make better decisions regarding their health care.

For the purposes of the Medicaid Incentive Program, eligible professionals, eligible hospitals and critical access hospitals must use certified EHR technology. Certified EHR technology means providers and patients can be confident that their systems are secure and can maintain data confidentially, as well as work with other systems to share information.
