Description:

The CHIN is a collaborative effort of local healthcare providers to operate a shared electronic network that securely collects and exchanges data on patients’ health status and access to services. Participating providers have real-time, secure electronic access to patient data, and use this information to verify patient diagnosis and treatment information, make care management decisions or referrals, and avoid unnecessary and duplicative procedures.

Providers access the CHIN network via the Internet, using secure authentication and verification procedures, and only participating providers can view patient information. Patients consent to have their information added to the CHIN system and shared with participating providers. CHIN policies and procedures comply with federal (HIPAA) and state (Florida) laws regarding protection of patients’ health information.

The software that serves as the basis for CHIN operations has been upgraded and we added customized functions to improve data security, matching/un-matching of records, and automated CCR interfacing.

A new customized feature of CHIN is the electronic We Care system which enables We Care program coordinators in both counties to track patient eligibility, enrollment, treatment, referrals, and measure the value of specialty care services donated to poor and uninsured patients. CHIN partnered with the Escambia and Santa Rosa Health Departments to develop e-WeCare as a service to local health care providers and community leaders.

The CHIN provides a centralized and reliable source of information about patients in care or seeking treatment from local providers. CHIN targets adult patients who are poor and uninsured and have chronic health conditions requiring regular medical care who seek services through the community safety net that includes hospitals, clinics, private practitioners and specialty care providers. CHIN has capability to track all patients, but currently focuses on ambulatory care. The CHIN system also tracks community fulfillment of requests for no-cost prescription medications for eligible patients, so it includes records on limited number of patients who may be eligible for Medicaid.

The CHIN is a community health resource, but it does not provide direct services to patients. One goal of the program is to reduce unnecessary use of Hospital Emergency Departments as primary care access points and assist uninsured patients in finding a medical home. CHIN is a project of AccessEscambia and the Partnership for a Health Community, nonprofit organizations established to improve community health and access to care for the people of Northwest Florida.

Community providers participating (utilization rates vary) in health information exchange and data sharing through CHIN include: Baptist Hospital, Cardiology Consultants PA, Circle Community Clinic, Escambia Community Clinics, Escambia Dental Cooperative, Escambia (County) Health Department, Gulf Breeze Hospital, Health & Hope Clinic, Jay Hospital, Lakeview Center (all sites), Mental Health Association WFL, Pensacola Junior College Health Professions, Sacred Heart Hospital, St. Joseph Medical Clinic, Santa Rosa Community Clinic, Santa Rosa (County) Health Department, West Florida AHEC, and West Florida Hospital.
