The Risk Management & Patient Safety Program (Program) is housed in the Florida Center for Health Information and Policy Analysis.
The Program is charged with receiving and reviewing Code 15 Reports and Annual Reports from Hospitals, Ambulatory Surgical Centers, and HMO’s; and both the 1-day and 15-day Adverse Incident Reports from Nursing Homes and Assisted Living Facilities. The Program is staffed by a risk manager, two nurses, a research associate and data entry staff.

The Program staff will conduct in-depth analyses of the reported incidents, requesting additional information, if needed, to determine what happened and how the facility responded to the incident. Program staff work closely with risk managers to identify the best practices implemented in response to an incident, and what actions were taken to prevent the incident from re-occurring.
By working closely with risk managers from around the state, valuable information will be able to be shared across facilities. All information shared between the Program, risk managers and the facilities around the state will be de-identified.
