Disease Management, Lifestyle Campaign and Diet Intervention

Brief Description of Goals of Contract:

In recent years, the Florida Medicaid program has implemented several disease management programs to reduce the complications of certain diseases.  Efforts have concentrated on diseases which have known treatments that lead to reductions in costs and or better outcomes for beneficiaries.  The Department of Nutrition, Food and Exercise Sciences at Florida State University are testing several approaches that could result in improved outcomes for Medicaid beneficiaries.  All three are targeted toward reducing obesity through exercise or better nutrition.

A. Exercise Management for Overweight Middle-Aged Women

A home-based exercise intervention for overweight middle-aged Medicaid beneficiaries will demonstrate whether a walking program is cost effective for this Medicaid population in reducing health risk factors for coronary heart disease. 

B. Lifestyle Campaign

The purpose of this project is to determine if beneficiaries of a mail-delivered instructional intervention on the importance of physical activity including how to increase daily physical activity will result in reduced Medicaid expenditures for the targeted group.

C. Diet Intervention

A disease management approach called Cookbook will be demonstrated on overweight African American Medicaid eligible women who are served by a Community Health Center to determine if women who are offered the opportunity to participate in the project lose weight, have lower medical costs in the 18-month period from the start of the intervention than does a similar group at another health center who will be given traditional advice regarding the need to lose weight and exercise more.

Physical Activity Questionnaire Statewide survey sent to Medicaid beneficiaries ages 40-65 who are not enrolled in a disease managemnt program.Beneficiaries responding have been randomly assigned to a control group or intervention group.  Follow-up survey will be completed with physical health status results being compared.

"Living An Active Lifestyle" A pamphlet (available in Spanish and English) provided to Medicaid beneficiaries participating in the lifestyle campaign intervention group.  This booklet emphasizes the importance of regular exercise and includes tips on how to get started, the importance of remaining active, staying healthy, and tips on maintaining or losing body weight.

"Modified Soul Food Cookbook" The recipes were produced and tested in FSU Nutrition Program Foods Labs by a group of neighborhood women organized into a club called The Modified Soul Food Club.  The cookbook is used in conjunction with the Medicaid diet intervention program involving middle school students and adults.

"Health-Related Characteristics of Overweight and Obese Women on Medicaid(Baseline and Preliminary Data) Presentation of baseline and preliminary information.  The purpose of this section of the study is to evaluate the health and physical activity levels with pedometers of overweight and obese, lower socioeconomic women 40 to 65 years of age.

"Daily Physical Activity in Lower Socioeconomic Status Middle-aged Adults" Presentation of baseline and preliminary information.  The purpose of this section of the study is to determine the amount of physical activity in lower socioeconomic status middle-aged adults.  Physical activity surveys were mailed to a random sample of 10,000 adult Medicaid recipients statewide.
