Statewide Medicaid Residency Program

Below is the required submission paperwork for participation in the SFY 2015-16 Statewide Medicaid Residency (GME) Program.  In order to participate, the providers are required to complete and submit both the GME FTE Template and Attestation form to be considered for inclusion in the program.  The deadline to return both documents to the Agency via email is August 18, 2015. You may send completed GME FTE templates and Attestation forms to LIPProvidersReports@ahca.myflorida.com

MMA Waiver Extension Request (2014-2017) of the 1115 Research and Demonstration Waiver

On November 27, 2013, the Agency submitted a three-year waiver extension request to Federal CMS to extend Florida's 1115 MMA Waiver for the period July 1, 2014 to June 30, 2017. Please refer to the Federal Authorities webpage for more information on the waiver extension request.
