Recipient Notice:

You have recently lost your Medicaid/HMO coverage and now may be eligible for a special Medicaid program that provides coverage for family planning services only.

You must meet the household income requirement to be eligible for this special program for family planning services.

This family planning coverage may be available to you for up to 2 years if you apply now!

You will be able to get the following services under this program:

  • Physical exams including a pap smear;
  • Birth control supplies such as pills, depo (the shot), condoms and diaphragms; and
  • Pregnancy tests and limited treatment for sexually transmitted diseases.
Mothers and babies are healthier if mothers wait two years between pregnancies so apply now for this coverage.

How to apply:

  • Completely fill out the enclosed application;
  • Mail or bring the application, pay stubs, and this letter to your local county health department;
  • Your county health department will notify you if your application is approved or denied;
  • Call the Family Health Line at 1-800-451-2229 if you need an application in Spanish or Haitian Creole, or if you have any questions.
