Registration: Eligible Professionals & Hospitals

Eligible professionals are required to register on the national level through the CMS website prior to completing the state application.


Step 1: Register on the Medicare and Medicaid EHR Incentive Program Registration and Attestation System (R&A).

Eligible Providers will need to complete the following in order to complete registration:

Eligible Hospitals will need the following information to complete registration:

  • CMS Identity and Access Management (I&A) User ID and Password
  • CMS Certification Number (CCN)
  • National Provider Identifier (NPI)
  • Hospital Tax Identification Number

Program Year 2016 was the last year providers could begin participating in the EHR Incentive Program. Though providers can no longer register to begin program participation, changes to their registration e.g. payee information must occur through the Medicare and Medicaid EHR Incentive Program Registration and Attestation System.

Hospitals that are eligible for EHR incentive payments under both Medicare and Medicaid should select "Both Medicare and Medicaid" during the registration process.


Step 2: Providers attesting to the program must complete the state level application known as MAPIR , which is accessed through the secure provider portal.The application link can be found under QuickLinks.
