“Our application did address how we have improved our measures over time to be below the state and national averages to include our pain measure, restraint measure (as we are restraint free), skin etc. I believe that to describe our “best” best practice would encompass all these accomplishments by highlighting our dynamic team collaboration and success. The culture of the Baldomero Lopez team is to always find ways to improve “what” and “how” we do things. It is truly a journey of continuous quality improvement. The Baldomero Lopez team is never afraid or shy about change to improve the services, care and quality of life for our veterans/residents. We are always excited about positive outcomes for our residents and creating a culture and environment for our residents of self esteem, independence and well being. Our mission is veterans/resident advocacy which the team is determined to fulfill as our ultimate goal.”

“We have two "best practices" we are very proud of at Bay Village. Our dining service scored 94% Resident and Family Satisfaction with our program. It emphasizes choice and individual preferences to raise Resident Satisfaction.

Comprehensive end-of-life care is the other best practice we have developed over the past 5years. When a resident is actively dying we relocate their roommate and the family is able to remain in the room 24hours a day if they choose to do so. We also provide "angels" to sit with the Resident if there is no family or to provide some respite for the family members. We have a "comfort cart" with a variety of items: Down pillow and comforter, lotions, portable stereo with appropriate CD's to provide soft music. There are books, hard candy and items for the family members to use. Our chaplain and the Religious Life Committee visit frequently to provide support and pray with the family. We have an angel statue that is lit at the bedside to alert the staff, volunteers, visitors etc. It reminds everyone to check frequently on this resident and family and provide the support they need at this critical time.”

“My favorite Best Practice is our Restorative Volleyball Program. This was initiated by two staff members and has become the most well attended exercise program we have. Staff and residents alike enjoy participating in this activity. Residents play volleyball with a beach ball and a net. Residents are on one side and restorative nursing staff are on the other. Residents improve their range of motion, sitting posture, hand/eye coordination and overall countenance. Once a month the residents challenge staff to a game. Staff from all departments including our CEO come and play the residents. The one rule for staff is they must remain in their chairs. If they rise up from a chair, a lap buddy is placed on them. This activity is the highlight of many resident's days. We have had residents refuse to go out with family because they wanted to practice for the up-coming match with staff. One resident told his daughter if she wanted to see him, she'd have to come to practice because there was no way he was going to miss volleyball. The additional benefit has been that residents interact with one another more. They will call out to another player to get the ball or will give a "way to go" to another resident by name.

The other adopted best practice which I am very proud of is "The Mom Principle". It states that "Care for a resident the way you would want your mom cared for". We also take it one step further to say, "How would you mom feel about the way you are caring for the resident." We feel this principle promotes compassion and quality care.”

“The Mayflower Remembers” is our unique method of saying ‘goodbye’ when one of our Residents dies. Day-to-day care giving tasks naturally foster closeness and strong emotional attachment to Residents. The Mayflower, in fact, further encourages this relationship by making permanent caregiver assignments. When a Resident does expire, the Resident’s families sometimes have a service locally, and staff may choose to attend. However, many times, services and burial are planned at great distances from The Mayflower Health Center. This circumstance gives the caretaker no opportunity to pay their respects or bring closure to this meaningful, intimate relationship.

“The Mayflower Remembers” affords our staff an opportunity to say ‘goodbye’ in a meaningful way. A memorial white rose with white ribbon is hung in the employee break room at the time clock to notify staff of the loss shortly after a Resident dies. Within 24 hours of the Resident’s passing, a Mayflower Remembers notice is posted notifying family members and staff of the gathering. Family members are encouraged to attend, but are always welcome and seldom present.

A very large bouquet of long-stemmed roses is purchased and arranged on the table in the meeting room. Oftentimes other memorabilia of the Resident is placed along with the roses. This informal reminiscing session is facilitated by the Director of Social Services or the Director of Nursing. The meeting is opened by the facilitator, who offers pertinent data about the Resident’s life. The staff then reminisces and is encouraged to talk freely about the Resident – sharing the good, the sad and often funny memories. A rose of remembrance from the bouquet is given to each attendee at the end of this service to honor the passing of this special person that we loved and cared for.

The staff has expressed gratitude for this opportunity to better deal with the ever-present and sometimes frequent losses. Of course, the longer a Resident is with us, and the more charming and engaging one is, the more difficult for us to let go. We are convinced that this program helps our staff to cope successfully with these inevitable losses, and thus makes them respond to our Residents with even greater respect and dignity.”
