The Agency is unable to accept applications submitted by other state agency that are not accompanied with a copy of the CNPPPJT2 from the State's FLAIR Accounting System. Any application that is not submitted with a copy of the CNPPPJT2 will be returned to the applicant.

The Agency cannot accept journal tranfers, checks or money order for payment of federal CLIA fees. All CLIA fee payments must be submitted directly to CMS CLIA Program, Post Office Box 70948, Charlotte, North Carolina 28272-0948. All payments must be made by check or money order and made payable to the CMS CLIA program.
