Information Required to be Posted at Licensed Clinics
- Sign identifying the medical or clinic director must be posted in a location visible to all patient. Subsection 400.9935(1) (a),Florida Statutes.
- Notice of Insurance Fraud Reward Sign is required to be posted in all licensed clinics in accordance with s. 400.9935 (9), Florida Statutes: Notice of Insurance Fraud Reward Sign.
A licensee must inform the client and his or her immediate family or representative, if appropriate, of the right to report.
[s. 408.810 (5)(a), Florida Statutes]:
- Complaints. The statewide toll-free telephone number for reporting complaints to the agency must be provided to clients in a manner that is clearly legible and must include the words: "To report a complaint regarding the services you receive, please call the Agency for Health Care Administration toll-free (1-888-419-3456)."
- Abusive, neglectful, or exploitative practices. The statewide toll-free telephone number for the central abuse hotline must be provided to clients in a manner that is clearly legible and must include the words: "To report abuse, neglect, or exploitation, please call the Florida Deaprtment of Children and Families toll-free (1-800-926-2873)."
- Medicaid fraud. An agency-written description of Medicaid fraud and the statewide toll-free telephone number for the central Medicaid fraud hotline must be provided to clients in a manner that is clearly legible and must include the words: "To report suspected Medicaid fraud, please call the AHCA Medicaid Program Integrity toll-free (1-888-419-3456)."
