Health Care Licensing Procedures Act

New Licensure Information Necessary Effective October 1, 2006. Forms to submit with all licensure/registration applications: Health Care Licensing Application, Addendum, Voluntary Board Member Affidavit and Affidavit of Compliance with Level 2 Background Screening for Covered Employees (AHCA Form #3100-0007). In addition, the Affidavit of Compliance with Background Screening Requirements (AHCA Form #3100-0008) shall be submitted with proof of compliance with Level 2 screeening standards within the previous 5 years as provided in subsection 408.809(2), Florida Statutes. These forms are recommended for use beginning October 1, 2006, to comply with the reporting requirements pursuant to Chapter 408, Part II, Florida Statutes. Chapter 408, Part II, Florida Statutes

With the enactment of 2006-192, Laws of Florida (House Bill 7141), changes in health care facility licensing will begin in Florida on October 1, 2006.

From the providers’ perspective, these changes involve biennial licensure, renewal reminder notices, uniformity in time frames associated with applications and less variation in basic licensure processes between provider types.

From the Agency’s perspective, these changes involve streamlined processes among provider types and greater use of automation and consistency across the 30 types of providers regulated by the Agency.

Regulated providers received a notice from the Agency in September 2006 including key changes effective October 1, 2006.
