Out of State Applicants

The Livescan Service Provider Fieldprint has many locations outside of Florida with Livescan and photo capability to have the screening entered into the Clearinghouse. Please visit Fieldprint to see if there is a convenient location: Fieldprint Florida

It is required that you initiate each new screening on the Clearinghouse Results Website here: Clearinghouse Results Website AHCA Portal Login

By initiating screenings you not only will receive email notifications once the results are available, but you will also be notified if the applicant is subsequently arrested and/or has an eligibility status change.

If you are applying for licensure with the Agency, please follow the steps below:

  1. Submit Initial Application for Licensure to Agency for Health Care Administration
  2. Receive AHCA Provider Number once application has been entered into system for processing
  3. Create user account on the Clearinghouse Results Website AHCA Portal Login
    1. Only users responsible for handling background screening for the application should create an account
    2. Each individual requiring a background screening does NOT need to create an account
  4. Request program access to Background Screening Clearinghouse – Agency for Health Care Administration and generate user agreement
  5. Submit signed Provider User Registration Agreement to the Background Screening Unit for approval
  6. Received approval (or denial) email within 1 – 2 business days
  7. Once approved, log into the Clearinghouse Results Website
  8. Search for eligibility determinations or request new screenings for required personnel
  9. Add employment history for Eligible personnel

If you are applying for an initial Medicaid Provider ID, please follow the steps below:

  1. Submit Initial Application to Medicaid Provider Enrollment
  2. Receive ATN (application must be fully submitted to Medicaid Provider Enrollment)
  3. Create user account on the Clearinghouse Results Website AHCA Portal Login
    1. Only users responsible for handling background screening for the application should create an account
    2. Each individual requiring a background screening does NOT need to create an account
  4. Request program access to Background Screening Clearinghouse – Florida Medicaid and generate user agreement
  5. Submit signed Provider User Registration Agreement to Medicaid for approval
  6. Received approval (or denial) email within 3 ‐ 5 business days
  7. Once approved, log into the Clearinghouse Results Website
  8. Search for eligibility determinations or request new screenings for required personnel
  9. Add employment history for Eligible personnel
