Care Facility Information Do you need to locate a particular health
care facility like a Nursing Home or Hospital? This link takes you to
our companion site
www.FloridaHealthFinder.gov where you can search and locate a wide variety
of health care facilities. In addition, it's mapping function can even
plan the route from your home to their front door!
- Every competent adult has the right to make decisions concerning his or her own health,
including the right to choose or refuse medical treatment. AHCA has compiled three types of advance directives:
a Living Will,
Health Care Surrogate Designation and an Anatomical Donation. You might choose
to complete one, two, or all three of these forms. The pamphlet provides information to
help you decide what will best serve your needs.
- The HIPAA Compliance Office assists Medicaid beneficiaries
in exercising their rights, including requests for claims records, handles HIPAA complaints against
the Agency and advises AHCA on how to comply with HIPAA.

This site provides you with comprehensive information about nursing homes. It also
has a searchable national data base of nursing homes by state. Here you can compare
nursing homes by various categories. Locate nursing homes by county, city, zip, or
name. Search for information about the nursing home, about the residents in the
nursing home, about the nursing home inspection record, and about the nursing home staff.
- The Agency's Division of Health Quality Assurance licenses and regulates most of our health care facilities in Florida.
Hospitals, Ambulatory Surgical Centers, Nursing Homes, Adult Living Facilities and Home Health
Agencies are just a few.

- Subscriber Assistance Program (SAP) is a program designed
to provide unbiased assistance to consumers of HMOs with their unresolved grievances.
- Managed Care Ombudsman Committees Program (PDF 1.5 MB) are voluntary organizations that serve as advocates for consumers with their managed
health care plans.
